Hide Content Behind a Poll

I want to offer a free sample of my company’s product in exchange for filling in a simple survey to gather some marketing data. Adding a survey seems simple enough, but, in order for this concept to work, I need to make sure visitors can’t access the free download without filling in the survey. Can anyone recommend a plugin that would accomplish this and how to set it up? Thanks!
