Your form includes only two equations, the first one: `fieldname9` that is working properly, and the second equation: * `fieldname5,fieldname11,fieldname12` * That is incorrect. If you want to concatenate fields you should use the “CONCATENATE” operation, as follows: * `CONCATENATE(fieldname5,fieldname11,fieldname12)` * The second error. You are generating input and textarea tags manually into the“ HTML Content” fields, these fields cannot be used from the equations. Furthermore, you are generating a conflict because you are duplicating the ids of the fields. * Best regards.
