Header.php or theme-editor.php hacked?

    it’s been a long time that i encountered with 404 error when I update my header in theme editor.
    after that i encounter with a text “hacked by …”
    i restore my backups and i still have problems of 404 error page when i only update header.php

    Could any one help me in this issue?
