Header not displaying on all pages & remove index.php from URLs

Starting a site for an upcoming conference and I mistakenly thought WP would be easier than other tools. Having problems!
1. My header banner is only displaying on the home page, and not the other WP pages.
http://focs2020.cs.duke.edu/index.php/program/
http://focs2020.cs.duke.edu/index.php/registration/2. How do you get rid of the “index.php” in the URLs – really dislike that. Would prefer the URLs to be this – with NO index.php in the URL:
http://focs2020.cs.duke.edu/program/
http://focs2020.cs.duke.edu/registration/Thank you.
