There is a parameter called ‘uid’ which is used to pass your Feedburner ID to the plugin. If you do not enter a default on the plugin settings page, you need to pass this value to the plugin in the shortcode (or in your PHP code that calls the function to display the form).
As genobz already pointed out, you do this by passing the ‘uid’ parameter with the name of the FB feed you want to link to the form. You should be able to do this on a per-shortcode basis, so you can link different feeds to each form.
