I think that the email problem is not from this plugin. It doesn’t send out any email, except to the admin.
The plugin will lock out sysops who appear to be spammers, so there is definitely an issue with getting locked out of a site. If you seem to be a spammer, either by behavior or from being reported to one of the spammer sites, then you should avoid this plugin.

Keith: Respectfully, how would we know if we, as a siteadmin, would fall into this category? I don’t want to be locked out of my own website.

We’re running about 98% caught fake registrations, but the bots seem to be adapting. Nice job nonetheless.
I use this plugin on over ten sites and I have never had many issues with it. The plugin has many configuration options. Usually adjusting them correctly resolves most issues.
