    It looks like the Javascript isn’t working on your page. What theme are you using? Can you try using the default WordPress theme to see if that fixes the problem? If so, I’ll see if I can find out what the issue is with your theme.

    If that doesn’t fix it, then there’s a conflict with one of your plugins. If you could disable them one by one to see which one is the problem, then I can try to find out what the issue is with the conflicting plugin.

    Hi Dave,
    I’m getting exactly the same with our site at: http://www.life-cc.org/gifts-survey

    We’re using the theme “Churchope”.

    I don’t think there’s a problem with any plug ins, but could be wrong.

    Any light you can share would be really appreciated.
