Please use a standard wordpress theme to test and debug.

it says on the setting page “Select a page with the booking form”. How I regognize such a page and how the booking form is put on this page?

As soon as the plugin is activated some page are automatically created ( an alert informs you on that).
Among these page there is one called “booking” with a shortcode [salon/] inside.
Anyway you can place this shortcode where ever you want.
OK – the page works – but wht ever i trie the front end has huge distances between lines and now font on the buttons. It also does not reflect the settings I choosed in the settings.Like Booking range 2 hours 3month
