Sure thing, just add a text / hidden field to your MailChimp list (in MailChimp) and then include this in your form. Let’s say we name it SOURCE. In that case, the HTML would look something like this.

<input type="hidden" name="SOURCE" value="{current_url}" />

The MailChimp for WordPress plugin will automatically replace {current_url} with the current page URL.
