free form maker with PayPal integration???

Are there any plugins out there where I can create a form and use PayPay without paying any fees?

I already have paypal integrated, but now he wants to add a form as well. So the customer would fill out the form, select which plan he wants, and then be redirected to paypal. If I can find a free way of doing this without having to ask him to pay, that would be great!
