Formstack toolbar button goes to 404

    I click on the Formstack button in the toolbar when editing a page.
    A small popup appears; the contents of the popup is our 404 page.

    Browser console:

    GET https://OURWEBSITESDOMAIN.com/wp-content/plugins/formstack/tinymce/dialog.php 404 (Not Found)
[tinymce.min.js?ver=4208-20151113:3 GET]

    This happened in WP 4.2.5 as well.
    I confirmed that the file does exist on the server.
