The embed code uses Javascript to show the embedded form and if there are any other Javascript errors on the page before the form loads, those errors will keep our script from running correctly. You need to troubleshoot your page to see what’s keeping it from displaying.
I would remove the embedded forms and make a new simple page with the embedded forms to make sure you can load them with your template, then start re-adding the elements to see what is causing the problem.

Thanx a ton @alexpowers – much appreciate the response. I will need some more help please – On a new page the form is appearing, however not on the specified form.
Which is why i turned on the script debugging & this is the error message appearing in the console: Uncaught ReferenceError: EmbedManager is not defined
Can you please advise ?! Much appreciate all the help in advance !!

The issue is that there are other Javascript errors on your page that are occurring before our EmbedManager script is run. The other errors prevent following scripts from running, so your form doesn’t show.
You need to fix the other errors on your page so the Embed script can run.
