 * I’m looking for a forms plugin that can I can use to set up a table for data 
   input. Below is a link to an image that shows what I’d like to be able to have
   people fill out on the form:

 * I’m converting an existing animal adoption form for an animal rescue charity 
   organization into an online format, so people can fill in the adoption application
   form and submit it on the website. The form information should be sent via email
   to the charity group and the info laid out in an easy format for them to review.
 * Does anyone know of a forms plugin that can do this?
