Forms not submitting every time

Fairly randomly, users will try to submit a form, onclick of submit button they will stay on the same page and the form will stay filled out, or be dumped blank and the submission will not go through. This happens about 3/30 times with a variety of users on different machines – not always the same user. They see no error messages when it does not work. When it does not work, they see the url appending to the form on the page again (/#sf_form_salesforce_w2l_lead_4_sidebar). When it successfully submits users see the correct success url (/#agent-form) and success message on page.
We have been unable to reproduce in any way that makes sense – it seems completely random that the submissions are failing (tried different users, different machines, different networks, and different environments (dev, q, stage). Has this happened to anyone else?

The success redirect is currently Javascript based (it’s a long story of legacy code and no time to refactor).
I suspect the times it’s breaking are when something else has stopped JS execution on the page, so the success redirect never fires.
At some point, I’ll have time to refactor the submission/redirect to PHP. Until then, I’d recommend using an error monitoring service like Rollbar to track down your intermittent JS issues.
