forms: how to show a list of items the user enters?

hi,

i need to build a form in ehich the user will select the number of item from a dropdown list, then enter the name of the item in a textbox, and the press a button that will display the items he had writen in the text box with the item he selected in the drop down list..

hope its clear enough 🙂 if not, how can i upload a picture of my intentions?

i am using FormBuilder Plugin or FormManager Plugin..
