form plugin with a difference required

Hi all, im looking for form a plugin with a difference for my WordPress site. Its a bit hard for me to explain what i want but here’s an example of how i want it to operate. Im not even sure such a thing exists or if so, what its called.

Hers an eg of what i want:

Say if i have a sun tan website and i promote a question on Google “find out how long it takes for you get a nice tan?”, they fill in a form and .

They then click to go to my site and answer a series of 5 questions with say 4 options each, they then receive an email with an answer that is determined by the options they chose:

questions are:

1: Which part of the world do you live in? (pick one: Africa, America, Europe, Asia)

2: What color skin do you have? (pick one: white, light brown, medium brown, dark brown)

3: How much time do you spend in the sun? (pick one: 1 hr, 2 hr, 3 hr, 4 hr)

4: What time of day is it? (pick one: 10am, 11am, 1pm, 3pm, 5pm)

These answers (each variation) correspond to a certain answer that will be emailed to the web site visitor after he enters his name and email address.

So if someone answers Africa, light brown, 2 hrs, 11am…he will be sent a a certain answer.

If he answers Asia, Dark brown, 1 hr, 5pm he will receive a different answer.

I want to be able to collect the email addresses of visitors to the site.
