 * I am hoping someone can guide me in the right direction.
 * I want to create a form that asks the questions and on the basis of those questions
   gives a specific result.
 * Example of this would be:-
    Surname Date of Birth Post Code
 * These would give the date of expiry of insurance as an example.
 * What would I need to achieve something like this?
