# Form not working

 * Hey All.
 * So I bought gravity forms and also installed this plugin. When i hit “Apply” 
   I still get the “Send an email to…” and not the form. I have setup everything
   as per demo video.
 * Any help?

 * Is there a fix for this yet? I need to know where to insert the code, what the
   proper code is, and anything else that needs to be create like hidden fields.
