Form not showing

    Hi! Let me tell you first that I am really looking forward for this excellent plugin.
    I have installed the 2.0.4 version on my Engrave-lite theme. The FAQ is showed but the only box available is “Your Question”. It doesn’t show, name, last name, email. I also sent a question and I didn’t receive the email with the notification. It also doesn’t show on the tab.
    I am very new on wordpress but I will really like to include this future on my blog. Thank you!
