Form Maker Not sending Email or Being logged in Submissions

Hi,
I have used Form Maker for about 6 years from first developing our Website.
After updating both WordPress and the theme last year I began noticing that Membership applications were not being received via Email. I discovered that sometimes after the form was submitted it would not progress and display the completed page and would consequently not submit or send the form via Email. I discovered that if the form was subsequently re submitted and the confirmation page was displayed all was well and the form would be submitted and an Email sent to the recipient set in the Email setting.
I tried everything I could think of to fix the problem to no avail, so I decided the best thing was to add a notification above the form to notify anyone filling in the form to make sure they got the confirmation screen, and if not to re submit the form until they got the confirmation screen. Thought that was a suitable work around.
I have now noticed that not only are no forms being emailed but are not even showing on the submissions page of the pluggin even though the option is enabled in settings.
I have updated the pluggin to the latest version but has made no difference.
Any suggestions would be appreciated.
