Form Layout: Alignment of Input Fields

    The answer to my question is probably rather simple, but I have been trying for a long time and I didn’t find solution.
    This is my form: http://www.mygermanphone.de/request-a-free-sim-card/
    I would like to align some fields of my form in the same line (row), for example “First Name” and “Last Name”. Or some fields from the address section. But how do I do that? How can I get the fields First Name and Last Name to be next to each other? Every input field only appears to me in a new line.

    Another question would be how I could make all of the text and form disappear once the form was sent successfully and only display the success response message. Can that be done with Javascript? And what would be the code?
