 * Please could anyone help with a few issues that I have with a form.
 * 1, I need the radio buttons to be a required entry (happy for these to be check
   boxes if need be).
 * 2, I need the text boxes below each to be completed IF they answer yes to a question.
 * 3, I would like the check boxes to have placeholder text in there.
 * Here are the relevant code excerpts:
