Form is malfunctioning, nothing works.

I looked through the documentation and the support forum and wasn’t able to find this topic.
I just created a new form that I want to use for a secret page for my promotional clients. I wanted it to be a simple “I agree” form where they put their name and submit.
But upon making the new form and inserting into my page, none of the elements work. The checkbox doesn’t check. The text fields don’t respond. The Submit button doesn’t click. And the text from the submit button is duplicated and posted off the left side of the screen on the top line where the form starts. I thought I may have put in a faulty code and did some debugging, but then just decided to delete the form and insert a standard one to see if something was different. Nothing changed.
Not sure why this happened, but would like a bit of help as I’m thinking it could be a bug in the plugin code.
