 * So if I am getting this right you were using a form and it has disappeared after
   you have updated MailPoet.
 * Could you confirm which kind of form it is, and if it is a MailPoet form or generated
   with another plugin?
 * It would also be nice to know what is the MailPoet version that your site is 
   currently using.

 * Hi,
 * I’m using the Mailpoët plugin. In the back-end I use the Mailpoët widget.
    It
   used to work perfectly for months and now it’s gone (widget is still in the same
   place).
 * I am using version 3.71.2

   widget on your site’s dashboard?
