form is cut off

I am using wp-customer-reviews on Ultimate Product Catalogue.
But I have a problem to show the whole form.

I tried to longer description on http://test.bobydo.com/products/?&SingleProduct=2b and it has not been cut off.
I guess there is css setting needs to be change when the “Please put your reviews” is clicked.
Could you please help me with it?
