The email notification is configured with "email-attachment":"true" (which explains why attachments are working), but the email body template lacks the field variables needed to display the form data.

To fix this, the email template in the Forminator form settings would need to be updated to include the desired form field placeholders, or there may be an option in Forminator to “Include form data in email” or “Auto-generate email content” that needs to be enabled.

This is a common configuration oversight when setting up form notifications – the notification gets created but the actual form field data doesn’t get added to the email template.
