# Form fields & submission data do not load

 * I have been trying to understand what is happening for weeks now.
 * Seemingly randomly, a few forms (four of ten) just do not load data from DB. 
   The form field settings are blank (as if the form is empty), the submission data
   is invisible (the UI acknowledges X submission occurred, and what the ID is, 
   but the content of the submission is blank) and on the frontend, the form does
   not render (presumably because it cannot find the fields). I also notice when
   the issue has occurred, the form name changes from title case (School Application
   Form) to slug form (school-application-form) – almost like it is falling back
   to slug because it cannot find anymore details on the post/form.
 * However, when I look in the database, such as `wp_postmeta` table filtering for
   key `forminator_form_meta` I can see the forms and their fields data.
 * I can also see in the submissions & submission meta tables (`wp_frmt_form_entry_meta`,`
   wp_frmt_form_entry`) that the submission entries are also recorded perfectly 
   fine in the DB, just do not show in the WP Admin UI nor the frontend (i.e., to
   render the form).
 * I have to restore from backup, and then every so often (I think after a form 
   submission, but not sure) this same issue [of form not rendering] occurs again.
 * I have tried:
    - Disabling all other plugins and switching to default WP theme
    - Checking for and disabling any crons that look suspicious
    - Duplicating the affected forms and trying again
    - Adjusting the affected form slugs (via DB)
 * A staging environment with the issue after this “reset” occurs is available for
   you to look at if you want to poke around and see if you can understand why it
   cannot load the data from DB.
 * I am running WordPress 7.0.4, PHP 8.4, Forminator 1.57.1.

 * I’m sorry to hear you are having these issues with Forminator. Just to confirm,
   when you restore a backup of your site, and the forms are failing again, are 
   the same forms as before? the same 4 of 10 forms you have on your site?
 * Can you please share us an export file of each one of these forms? If possible,
   please also share us the pages where you display these forms.
 * You can get the export file by going to Forminator -> Forms, open the menu of
   the form and select “Export”. The window that appears has an option to save the
   export data into a file.
 * Please upload those files to any file sharing service like GDrive or Dropbox
