Form fields not appears

    Hi TopdiR,

    Take a backup of settings from your clone site(option available under Advanced tab), and try to restore it in your live site.

    Hello, I have sort my issue next way:

    1. Open form fields;
    2. Add third “dummy” field. Save;
    3. Form appears. Than I delete third field and save again.

    For some weird reason this helps me.

    That’s truly weird. We tried at our end, but didn’t find the issue.

    Hello. I am having the same issue with the plugin. I have tried adding and removing fields and still can’t get any fields to appear on the lightbox. Any ideas? The formatting seems to hold, the fields are the only thing missing.
