Form disappears as page loads

    Hi there,
    I’ve got the most recent version of the plugin.

    When I put the form on my page it disappears as the page loads. When I inspect element there are no fields in the form. There is something about the international phone number?

    I’ve used this plugin before I used the most recent update and it worked fine. Now not displaying.
