form builder not generating in my wp dashboard

The form builder does not completely generate for me. I can see the left side with the menu items to add fields but the right side with the actual form is not appearing. I did try changing the Script Load Method but no change. I suspect it is because I tried another calculator plugin prior to this as I installed the plugin on one of my other sites and it is working fine. I did run a restore to an earlier point (prior to installing the plugin) but I noticed the plugin still recognized a test form I created.
I even went into the database and deleted a few fields it created. Big fail.
I think your plugin is perfect for what I am trying to accomplish. Any words of wisdom for how I can make this work? I am an old coder (think y2k and Cobol) so understand programming but never really learned php so wonder if that is why I am stuck.
