Form before Poll

Hi. I have inserted a poll in one of my page. I have made different sections in the poll with have a group of questions. I have inserted the whole poll by the following code into the page.

[wpsqt type=”poll” name=”TQM Implementation”]

But i can’t do the following. Please Help..

1) Insert a form before poll so that the user name/designation be displayed in the results.
2) Section Names of the poll.
