Form appearing two times – the_content

I currently use the plug-in Insert Pages to put reading orders at the bottom of some of our posts. The problem is that both optin-forms & Insert Pages uses “the_content” so the form is appearing two times.
An example can be found at the bottom of the post here:

I even went in and Excluded all of the pages that I use with the Insert Pages plug-in. I entered in the pages: 12068, 8143, 10396, 8164, 8176, 8257, 8247, 8233, 8183, 8225, 8237 11075, 11242, 12616, 10418, 11877, 8341, yet that didn’t work either.
So then I found an old post where you tried to help someone with a similar problem:

And I tried to replicate the code you gave him by changing it to:
.shortdesc #optinforms-form1-container{display:none;}
But it didn’t work. I entered the code above into the Custom CSS box within optin forms.Any help would be more than greatly appreciated!
