Thanks for using Chosen!
When adding this option with Jetpack, I am able to see the checkboxes, so there must be something else preventing them from showing.
First, update to the latest version of Jetpack and Chosen if there are any update notifications available in the dashboard.
Next, try clearing your browser’s cache and your site’s cache if you use a caching plugin. An older version of the page in the cache might be showing from before the checkboxes were added.
Lastly, if neither of those works, try deactivating all plugins on the site besides Jetpack and see if the checkboxes then appear. You can then reactivate the plugins until you find the one causing the issue.

Thanks so much for your suggestions! I really appreciate that you took the time to respond. I’m not sure why the option did not appear before, I disabled the plugins, re-enabled one by one and I now see the follow option.
