 * Is there a way to take a scanned PDF and make into an online form so someone 
   can fill it out while on a website?
 * If so can someone show me how to make this possible?

 * No, computers aren’t that clever. You need to create the form structure in the
   PDF and then using code convert that into a form in your WordPress website, but
   it’s a serious job that should be done by a developer or three.

   file without a developer to assist?
