File Uploading and Attachment

I’ve read the section of the docs having to do with including a required attachment on a form, but I’m still confused.

In File attachments, I placed [ACT-scores] [SAT-scores] [Transcript] for the three attachments the applicant need to include. What I don’t know is what I need to place on the form the applicant completes OR where I need to place it.

If someone can point me in the right direction, I’d appreciate it.
