File Name Upload, Image Size, Price List Help

A few questions:
1. I am trying to upload my images with the file name of each image intact and showing as if it were the title. Currently my Headline from my metadata is what is showing as the Title.
2. What image size should I be uploading, because I am having a big problem with images not uploading at all and I believe it is due to images site.
3. Is there a way to create price lists and then have add ons for each item. For example, my 4×6 would be $4 and then my client could choose to add on a protective coating for $1. Right now, I have an unsightly and confusing long list of prices.
