Features image no longer showing

I am not sure what happened or when this happened but I am on V 1.4.2 of the plugin and it used to work fine. Now the thumbnails on my site are no longer showing the featured image. It is just showing a blank square. The images for the thumbnails is now http://evilshenanigans.com/wp-content/uploads/nelioefi-placeholder-240×240.png

If I go into the post I see a transparent blank image in the Featured Image box and I see the correct image in the External Features Image box.

If I remove the standard WP featured image then the thumbnail returns.

any suggestions? In the past I had an issue with this plugin not working correctly with my theme and I was given some code to add to my functions.php. I am not sure if this is related. The plugin has been working fine up until now.
