I can’t find a theme with that name. Where did you get it from?
Where did you get this theme from? I’m not seeing that particular one when I search the https://wordpress.org/themes repository. Have you tried asking the theme’s author about the issue?

My bad, the theme is “Active: organization” or just “Active” it is the first theme in the selection when I first open up the theme tab.

Hm, where did you get that from? The only theme I can find called “Organization” is only available for WordPress.com blogs.
Also, what is the URL of the affected site?
What James asked but with a minor twist: the only theme I was able to find was https://wordpress.org/themes/activetab/
What happens if you switch themes?
