Ok, let’s find out for Safari. The favicon generated by RFG is supposed to work on both iOS (iPhone/iPad) and Mac OS. In both cases, Safari don’t display any favicon in the tabs anymore but use it in bookmarks and “Add to home screen” (iOS only). So which platform are you testing, iOS or Mac OS? Where do you notice the absence of the favicon? In bookmarks maybe?

Your icons are declared correctly. However, there a first icon declaration nearly the top of the page:
<link rel=”shortcut icon” type=”image/x-icon” href=”” />
Two issues with this declaration:
- RFG’s plugin already does this, so it id duplicated.
- The
href
attribute is empty, so this declaration is somehow erroneous. Maybe this is because the icon does not show up in your browser.
This first declaration probably comes from your theme or another plugin. I advice you to browse your theme option and look for a “favicon” setting of some kind. If possible, disable this entry completely. Else, set it to the content of http://kamadoworks.com/wp-content/uploads/fbrfg/favicon.ico . The declaration will still be duplicated but at least browsers will always pick the right icon.
