 * Could you please deactivate “sexy-login” plugin and try it out again?
 * Do you need both plugins activated? I mean.. Users Ultra and Sexy Login?

 * We’ve updated a new version. However, i highly recommend you deactivating the
   another login plugin to avoid conflicts.
