Fast Secure Contact Form not displaying in widget

Hi all,

I have followed all the steps at http://wordpress.org/support/topic/plugin-fast-secure-contact-form-is-there-a-widget?replies=9 to install the Fast Secure Contact Form in a widget on my site at http://www.kellyhainsworth.com/new/.

However, all it displays is [si-contact-form form=’1′] even though I have added the script to the functions.php page in the theme. I am using Thematic – does anyone have an idea why it’s not working?
