[resolved] Failed custom structure permalinks on self-hosted centos6 with domain mapping (3 posts)

WP 3.2.1 on self-hosted centos6 with WP network sub-directory config and WP MU Domain Mapping Sunrise plugin configured. Domains hosted at godaddy and pointed at my centos6 server's static ip in an A record.

Custom permalinks display Apache/2.2.15 (CentOS) Server error "Not Found The requested URL /postname/ was not found on this server."
Everything else functions correctly and each unique TLD correctly accesses default permalinks /?p=123, /wp-admin/, etc.

Displaying info.php shows mod_rewrite is a loaded module.

Domain mapping instructed that I add the following to a .htaccess file. There was no .htaccess file until I created (644) and added the following in /var/www/html:
    RewriteEngine On
    RewriteBase /
    RewriteRule ^index\.php$ - [L]

    # uploaded files
    RewriteRule ^([_0-9a-zA-Z-]+/)?files/(.+) wp-includes/ms-files.php?file=$2 [L]

    # add a trailing slash to /wp-admin
    RewriteRule ^([_0-9a-zA-Z-]+/)?wp-admin$ $1wp-admin/ [R=301,L]

    RewriteCond %{REQUEST_FILENAME} -f [OR]
    RewriteCond %{REQUEST_FILENAME} -d
    RewriteRule ^ - [L]
    RewriteRule ^[_0-9a-zA-Z-]+/(wp-(content|admin|includes).*) $1 [L]
    RewriteRule ^[_0-9a-zA-Z-]+/(.*\.php)$ $1 [L]
    RewriteRule . index.php [L]
