Extremely flexible form creator

JotForm is a very flexible form creator that will allow the user a lot of modification options. I’ve been using it on my site for years and the few times I’ve had any problems or questions, they offer great customer service to help me out. I’ve been able to set up forms that automatically send just the fields I want to just the people I want, and there are multiple options for integration with your website. I highly recommend this plug-in. As another user mentioned, if you have any trouble using their site, you may need to try from a different browser. I’ve used IE10 and Chrome with no problems.
