 * This is an excellent, fully functional form builder. One of the best wordpress
   plugins. It opens endless possibilities. I use to create calculators of all sorts.
   It needs a little bit of javascript code knowledge, but even for newbies, the
   developer is very supportive, and the support forum has answers for everyone.
