Event registraion plus shopping cart

I work for a non-profit looking to create a WP website to register participants for an event with different divisions, register for one of three clinics, and pre-order shirts (t-shirt, hoodie, & sweatshirt).

I’ve tried Woo Commerce for the shirts, but I can’t find a way to register the participant to record them into the following categories below:

Example customer flow:
1. Registrant’s demographic info (name, adrs, age, rank)
2. Registrant’s division (based on age & rank) – select one of ten
3. Registrant’s competition choice (#1, #2 or both)
4. Order attire (t-shirt, sweatshirt, hoodie & size)
5. Clinic preference (Friday & Saturday, Friday Only, Saturday Only)

I was able to do this last year using Google Forms with PayPal on our Joomla site, but would like to incorporate into a new WP site.

I’ve reviewed and read several posts which suggest Woo Agenda with Woo Commerce or other combinations. However, they appear to be for more complicated transactions. I just have a weekend event.
