if you are able to see the login history listing table in admin, you can ignore this error.

Please follow these following steps and let me know if you are still getting this error:
    -delete the error log file
    -deactivate the plugin (Not Delete/Uninstall)
    -activate the plugin
    -check error log file

    You can ignore this error.
Actually on activating the plugin, I am deleting an index from my plugin table using the code drop_index($table, 'faulh_user_traker_index').
In your case, the index could not be found and hence it is logging this error.

I am sure that the plugin should be working fine now in your system.
