Error plugin Quote Source

what plugin exactly are you using and where did you download it from?

as the plugin seems no longer supported, you might increase your chance of getting some suggestions by posting a live link to your site to a page where the problem can be inspected.
from just a picture or screenshot, getting any help is most unlikely.

there was actually no option for your desired layout in the plugin (you can find the options page under dashboard ‘Plugins’ ‘Quote Source’…).
the one you liked was only a default output if there is only one quote for the text.

I edited the plugin code to enable your layout for multiple quotes.

to put the edited code into the plugin, you will need to goto
‘Plugins’ ‘Editor’
‘Select Plugin to Edit: Quote Source’
then edit the two mentioned files and replace the full code of each with the code from the pastebins

I can ony see three input fields which all three also show in the post/page.

I added an option into the code to set a max number of source quote fields, max 20;
