Error Logging Out/Trying to Access wp-admin

Whenever I try to access /wp-admin for my site or logout, I always get redirected to the page http://mysite.com/#login.

I’ve tried clearing my cache and cookies, which gets me logged out, but when I try to login to /wp-admin again it does the same thing.

Any help towards a solution would be greatly appreciated.
