    js file responsible for Email Subscribers form is not loading on your site and hence you are the getting the error.
    Please de-activate, delete and install a new version of Email Subscribers and then check if the issue goes away.

    If it is still not working then temporarily de-activate all other plugins except Email Subscribers and then check if it works.
