error 500 but attachment is sent

I have a form for currirulum with attachment, on windows server.
If I send the data from the form, the email is received and the attachment is present, but on the front-end there is an icon that runs indefinitely.
The same form if I remove the attachment code works well.
what can 500 error depend on?

Does not work with the original template and without a plugin
the mail with attachment works well
